To request a copy of your transcript for any previous Eastland-Fairfield full-time training programs, please fill out the Transcript Request Form. There is a $5.00 fee for each transcript requested. A check or money order should be made payable to: Eastland-Fairfield Career and Technical Schools.

Mail the Transcript Request Form and payment to:

VISA or MasterCard is also accepted. Be sure to include the credit card number and expiration date of the credit card along with the Transcript Request Form. Fax the form to the attention of the financial aid officer at 614.836.8164.
